I hereby acknowledge and agree that in consideration of being permitted to participate in sports programs or activities organized, operated or sanctioned by Catholic Sports, or its affiliates, (hereinafter called "CATHOLIC SPORTS"):

1. I release CATHOLIC SPORTS, its directors, officers, employees, volunteers, partners, sponsors, contractors, agents, affiliates, and facilities (referred to here-in collectively as CATHOLIC SPORTS) from all liability, and do hereby waive as against CATHOLIC SPORTS all recourses, proceedings, claims, actions and causes of action of any kind whatsoever, in respect to all personal injuries, illness, infection or property losses which I may suffer arising out of or connected with my preparation for, or participation in, the sports programs or social activities organized by CATHOLIC SPORTS, notwithstanding that such injuries or losses may have been caused solely or partly by the negligence of CATHOLIC SPORTS,

2. And I hereby acknowledge and agree:

a. that I am at least 18 years of age;

b. that organized sports programs or activities can be very dangerous, exposing participants to many risks and hazards,

some of which are inherent in the very nature of sports itself, others which result from human error and negligence on

the part of the persons involved in preparing, organizing and staging sports programs and other activities;

c. that, as a result of the aforesaid risks and hazards, I as a participant may suffer serious personal injury, property loss, or

even death;

d. that some of the aforesaid risks and hazards are foreseeable, but others are not;

e. that I nevertheless freely and voluntarily assume all the aforesaid risks and hazards, and that, accordingly, my

preparation for, and participation in the aforesaid sports programs or activities shall be entirely at my own risk;

f. that I understand that CATHOLIC SPORTS does not assume any responsibility whatsoever for my safety during the

course of my preparation for or participation in the aforesaid sports programs and activities;

g. that I have carefully read this WAIVER OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, that I fully understand same, and that I am freely and voluntarily executing same;

h. that I understand clearly that by signing, or digitally agreeing, to this release I will be forever prevented from suing or

otherwise claiming against CATHOLIC SPORTS for any loss or damage connected with any property loss or personal injury

that I may sustain while participating in or preparing for any of the above mentioned sports programs or activities

whether or not such loss or injury is caused solely or partly by the NEGLIGENCE of CATHOLIC SPORTS;

i. that I have been given the opportunity and have been encouraged to seek independent legal advice prior to signing

this agreement;

j. that I understand clearly that CATHOLIC SPORTS would not permit me to participate in any such sports programs and

activities unless I signed the WAIVER OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT applies to all the aforesaid

sports programs or activities whether occurring in the near or distant future, and that the terms of this agreement need

not be brought to my attention each time I participate in a sports program or activity in order to be effective;

k. that the term sports programs or activities as used in this WAIVER OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT

includes without limiting the generality of that term, the programs, leagues, and social activities as well as all other

activities, programs and events that are in any way authorized, sanctioned, organized or operated by CATHOLIC SPORTS;

l. that this WAIVER OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT is binding on myself, my heirs, my executors,

administrators, personal representatives and assigns;

3. Photo & Video Release. I hereby grant CATHOLIC SPORTS, and its photographers/videographers, the right to use my likeness

in any publication, both digitally and printed, without payment or other consideration. I understand and agree that any photographs/videos taken by CATHOLIC SPORTS are the copyrighted work of their photographers/videographers and may be altered, edited, copied, printed, published or distributed at their discretion or the discretion of CATHOLIC SPORTS.

CATHOLIC SPORTS Code of Conduct-"Sportsmanship Contract"

1. CATHOLIC SPORTS is a sports-based community open to all people, of any faith, or lack of faith, background

2. Participation in CATHOLIC SPORTS leagues emphasize fun community and growth in faith.

3. Every CATHOLIC SPORTS league encourages sportsmanship and safety.

4. CATHOLIC SPORTS leagues do not tolerate profanity, rowdiness, poor sportsmanship or dangerous play. Such behavior

can warrant ejection from a game or match

5. The CATHOLIC SPORTS director and officials, or their liaison, have the ultimate authority to eject a player from a game.

6. Any player, or non-playing spectator, who is ejected from a game will be suspended for at least one more game.

7. Any player, or non-playing spectator, who is ejected from CATHOLIC SPORTS a second time is subject to suspension from

CATHOLIC SPORTS indefinitely. Any suspension is at the sole discretion of CATHOLIC SPORTS.

8. A player or non-playing captain who has been ejected from a game can be asked to leave the premises by any official or

CATHOLIC SPORTS representative, if the behavior continues to be a distraction. Failure to leave could result in forfeiture of

the game for their team. Please be advised thatspectators whom were invited by players and captains should adhere to 'good

behavior" or they are subject to ejection from the premises as well.

9. Any physical attack during or following a play will result in immediate ejection from the game, the player will be asked to

leave the premises at that time and will be subject to review by CATHOLIC SPORTS for consideration of expulsion for the rest

of the season or even the total elimination of future participation.

10. No player is allowed to participate in a game or match under the influence of alcohol, or any other controlled substance

including marijuana. You waive your right to hold CATHOLIC SPORTS liable for any damages due to the consumption of alcohol and will be responsible for any liability and legal fees to CATHOLIC SPORTS from damages where you are found to be in

negligence.
